Important information about the PADI Medical Statement

Answer YES or NO to each question on the medical form. If you are able to go through all of the questions and answer NO to each one, just fill out the form and sign it. If you go through all of the questions and find that you must answer YES to one or more questions, then go to the corresponding box on the second page, if you still have a YES then you must take the completed form to your physician. The physician will complete PAGE 3 of the form in full.



If you have answered YES to one or more questions on the medical form and not received medical clearance from a physician, you will be unable to participate in any in-water training for any scuba course at Ocean Pro Divers. Please see our Change and Cancellation Policy for applicable fees.



Full registration in the program can only be completed with payment.  The course can be paid over the phone with credit card or in person with credit card, debit card or cash. Your spot in a course can only be secured after full payment for the course.


Learning Material

Once your course has been paid in full, we can release the online learning program to you.  If you don’t receive the learning materials 24 hours after payment let us know and we can resend them. Please complete the online learning material before your course start date.

Scuba Gear

Please come in at least one week prior to your course to be fitted for dive gear. Call the shop to see when a good time to come in would be. Generally Monday, Friday and Saturday are not good days because some of the rental gear is booked out for the weekend. 



We need a digital photo of you for your certification card.  Please include a head shot (file size no larger than 250kb).  Please do not wear sunglasses or a hat in the picture.



There is parking in front of the shop, as well as the visitor parking in the parking lot. You may also park in spots 202, and 203.

Change and Cancellation Policy


Cancellation - Medical
Our courses are non-refundable once booked. The only exception to this is course cancellations with 30 days advance notice AND for medical purposes. Course materials are non-refundable - please ensure you address any medical concerns prior to commencing the academic portion of your course.

If a course must be cancelled due to a failed medical we offer a full refund on the course tuition, less the cost of course materials. This refund is available only if we are notified in writing more than 30 days prior to course commencement. A physician-signed medical release noting the reason for cancellation will be required.

If cancellation due to failed medical is within 30 days of the course commencement or after commencing the course, no refund is available.

Please ensure you complete the medical documentation and discuss any concerns you have with us. Cancellations for non-medical reasons are not refundable at any time.

Transfer to a New Course - 30 days notice
We understand that unforeseen circumstances may prevent you from starting your course on the agreed course commencement date. Transfers may be made to another program as long as the new program start date is within the 12 months following your original commencement date. There is no penalty for this transfer as long as 30 days advance notice is provided.

Transfers to a New Course - less than 30 days notice
Transfer requests made prior to the course commencement but with less than 30 day notice will incur a $100 transfer fee payable at the time of the transfer. Once the course has commenced, there is a $200 fee to transfer to a new course payable at the time of the transfer.

These fees apply regardless of the reason for the transfer. Transfers may be made to another program as long as the new program start date is within the 12 months following your original commencement date. Please note: if you have a gap in your diving history of more than 3 - 6 months, you may be required to participate in a Scuba Tune Up (at an additional cost). This will be determined with you and your instructor and is based on your experience and skill level.

No-shows will be treated as a course cancellation with no refunds made.

Missed Sessions
We understand that students may miss a session due to illness or emergency. Should you miss a class/pool session or ocean session, you will be required to take a make-up session at an additional cost of $100 per class/pool session missed or $200 per day of diving. This allows us to bring in additional staff to work directly with you to ensure that the necessary level of mastery of the information and skills is achieved. Private sessions are also available to students who would like a little additional time or personal attention to fully develop their skills.​

If the shop cancels your course or your trip

If Ocean Pro Divers initiates a cancelation to various reasons that could include be not limited to: Instructor Illness, Instructor non availability, government shutdowns, pool closures, weather etc. Ocean Pro will make every effort to work with you to reschedule your course or trip in a reasonable time frame, or offer you a spot on an upcoming previously scheduled course or trip. If the customer is unable to attend the rescheduled options, a partial refund (determined by progress in the course) may be granted less any learning materials issued.
