Complete the appropriate course document if you will be participating in the Discover Scuba Diving, Bubble Makers or Seal Team experience. The Medical Statement is required ONLY if you have answered YES to one of the medical questions on the participation document above. Have the physician discuss the conditions to which you have answered YES and decide with you if scuba training is safe for you. The physician will complete PAGE 2 of the form in full. Bring the signed form to the staff at Ocean Pro Divers on the first session of your training. If you have not received medical clearance from a physician and have answered YES to one or more questions on the medical form, you will be unable to participate in any in-water training for any scuba course at Ocean Pro Divers.
