Required Course Forms

Students are required to complete three forms for each scuba diving course they participate in.

PADI MEDICAL STATEMENT:

Prior to participating in any course students must complete a medical statement. If you are able to go through all of the questions and answer NO to each one, just fill out the form and bring it with you to class.

If you go through all of the questions and find that you must answer YES to one or more questions, then you must take the completed form to your physician. Have the physician discuss the conditions to which you have answered YES and decide with you if scuba training is safe for you. The physician will complete PAGE 2 of the form in full. Bring the signed form to the staff at Ocean Pro Divers on the first session of your training.

IMPORTANT: If you have not received medical clearance from a physician and have answered YES to one or more questions on the medical form, you will be unable to participate in any in-water training for any scuba course at Ocean Pro Divers. Please see our cancellation policy on our Sign Me Up page.

PADI LIABILITY RELEASE:

This form is used to inform students of the potential risks of scuba diving. Please select the form associated with the course you are participating in.

PADI STANDARD SAFE DIVING PRACTICES STATEMENT OF UNDERSTANDING:

This form outlines the basic safe diving practices that a diver will be trained to follow. By signing this document when registering for a course, a participant is agreeing to abide by these practices.
